How to Submit a Claim
If you are involved in an assault, or are exposed to an offender’s blood and body fluids; send us your report. Please email an incident report with a supervisor’s approval, along with your full name, complete mailing address, contact telephone number, institution and the last four numbers of your Social Security number.
